We perform approximately 72,000 hemodialysis treatments annually. As a leading centre for Dialysis Services, we can provide you with the care you need for your lifelong well-being.

Learning to live with kidney failure is a big adjustment, for you and your family. We have put together information to help answer some of your questions about hemodialysis, our unit, and the services we provide. This information only covers the basics. Our team provides prenatal as well as postpartum wellness services as well as a range of services for children and adolescents. Hope Haven Medical Healthy Living Clinic provides care for seniors to help them age in place. Our goal is to help seniors obtain the best level of functioning possible while continuing to live at home, to help prevent injuries and hospital visits.

Our dedicated team of healthcare professionals can complete a review of medical and cognitive health issues for people over the age of 65.

Our team of experienced healthcare professionals is committed to providing dignified care for seniors in our community. We provide care for patients over the age of 65 years that have been referred for a variety of geriatric concerns such as:

• Recent changes in physical, mental or functional abilities; falls, impaired mobility, dementia, delirium, change in mood, decreased ability for self-care;
• Major changes in support needs, caregiver stress, institutional placement being considered; and
• Safety concerns – physical, psychological, social, and environmental; or a sudden increase in the use of health care services over the last 6 months.

Common concerns among seniors and their families are preventing falls and caring for seniors after they have fallen to ensure the best recovery outcomes possible. Two of our popular clinics are the Bone Health Clinic and Fall Prevention Clinic.

Our Bone Health Clinic ensures that patients with post-hip fractures receive a bone health assessment and intervention for the best healing outcome possible.

Our Fall Prevention Clinic is designed to improve senior safety. During this program, patients will receive a consultation with a geriatrician, a falls assessment by a physiotherapist, a group exercise program and a home exercise program.
